- Approval (Signature) must be obtained from the Director of Kulliyyah/Dept./Centre
- Please follow-up with Administration Office at least 2 days after submitting the form.
- All applications for booking must be made in 5 Working Days prior to the function date.
- Late application, incomplete form or application without proper approval will not be entertained.
- Any postponement/cancellation of the request must be informed and an new application needs to be submitted.
- Document(s) of the programme/programmes approval must be attached.
